Left atrial pulmonary capillary, and oesophageal balloon pressure tracings in mitral valve disease.

چکیده
In selecting patients for mitral valvotomy, the exclusion of mitral insufficiency by common clinical methods has proven unreliable. We endeavoured to determine whether preoperative " pulmonary capillary " pressures and tracings taken by means of a balloon in the aesophagus at the left atrial level would be of value in the diagnosis of mitral insufficiency. Pressure pulses were also obtained directly from the left atrium at operation; their characteristics in mitral stenosis and mitral insufficiency were studied, and they were correlated with the " P.C." and oesophageal balloon tracings, since the latter two are of value only in so far as they reflect hemodynamic events within the left atrium. To accomplish these aims, we set out to obtain all three types of tracing in each of a series of 30 patients who were operated upon for the relief of mitral stenosis. We relied upon the surgeon's finger to detect insufficiency. Both the size of the mitral orifice and the presence of a regurgitant jet were taken into consideration (Wynn et al., 1952). These criteria may be inadequate but must serve until better ones are devised.

SHORT TERM CLINICAL OUTCOME OF PERCUTANEOUS BALLOON MITRAL VALVULOPLASTY VERSUS SURGICAL CLOSED MITRAL COMMISSUROTOMY
BaIlon mitral valvuloplasty and closed mitral valve conunissurotomy were performed in 450 and 127 patients, respectively. Also, 254 patients with severe mitral stenosis were enrolled in a prospective randomized trial comparing the two procedures. Immediately after balloon mitral valvuloplasty, mean left atrial pressure was 11.38±3.54 mmHg, transmitral valve gradient was 1.8±2 mmHg, and mit...
متن کاملHigh-risk Coronary Artery Bypass Grafting and Mitral Valve Replacement in a HIV Positive Patient
Certain subsets of high-risk mitral valve patients are not suitable candidates for transcatheter therapies. The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...
متن کاملComparison of left atrial pressure and wedge pulmonary capillary pressure. Pressure gradients between left atrium and left ventricle.
Simultaneous records of wedged pulmonary capillary pressure and left atrial pressure have been made in I2 patients. The pressure changes in the left atrium preceded those in the pulmonary capillary bed by o-o6 to O0I4 sec. This delay, together with the damping of the wedged pulmonary capillary pressure, may result in a false appearance of an end-diastolic gradient between the left atrium and th...
متن کاملDirect left atrial pressure tracings and the degree of mitral obstruction.
Attempts have been made to assess the degree of mitral obstruction in mitral stenosis by analysing the diastolic part of indirect left atrial pressure tracings (Owen and Wood, 1955). These pressure tracings have been obtained using a cardiac catheter in the pulmonary artery in the " wedge " position, and available evidence suggests that direct left atrial pressure tracings are comparable to ind...
متن کامل[mitral Stenosis].
8. What is the pathophysiology of mitral stenosis? Increased left atrial pressure is necessary to push blood through a stenotic mitral valve from the left atrium into the left ventricle. Increased left atrial pressure is transmitted retrograde into the pulmonary veins and pulmonary capillaries and ultimately into the pulmonary arteries. It gives the patient a sensation of dyspnea. A left atrial...
